How To Do Correction Of Name Affidavit

 


An affidavit of correction of name is a document sworn by a person affirming his or her real name as opposed to a wrong one. It is an affidavit sworn at the registry used for the correction of a name and notifying parties concerned of the correction. 

A correction of name affidavit is often used when there is a

(a) Wrong name
(b) wrong spelling of a name
(c) Where all your names are not properly arranged,
(d) Where there is omission of a name. 

Correction of Name Affidavit Is Used In The Following Instances:

- Correction of name on BVN details or bank account details. Sometimes, you may want to do a bank transaction or open another bank account, and you will be told that the name supplied is different from that of your BVN and that an affidavit of correction of name is required to correct it.

- Correction of name on academic or school documents such as WAEC documents, WASSCE certificate, NECO documents, UTME/JAMB documents, First School Leaving Certificate, statement of result, university certificate, a transcript, or NYSC certificate. This kind of correction may be needed where there was an error in registration or when providing details that eventually appeared on the document or certificate and has to be rectified before being accepted. 

- Correction of name on your identification documents, like your national I.D. card, driver's license, international passport, or voter's card. 

- Correction of the name of your business or company documents, especially where there is an error from you or the Corporate Affairs Commission (CAC).

- Any other incident requiring correction of name affidavit.

Contents of an Affidavit of Correction of Name 

An affidavit of correction of name should contain the following structure and information:

(I) A heading, stating "Affidavit of Correction of Name."

(II) The opening paragraph should state your proper name (as it ought to be), address, occupation, religion, nationality, and gender. 

(III) In full, state the wrong name you want to correct and the grounds (what happened, how it happened, and document it happened).

(IV) State why you now want to correct the name and that the proper name should be _____.

(V) Include that all previous documents bearing the incorrect name remain valid. 

(VI) Signature.

(VII) Passport photography (not in all cases or places)

It is very important that all these details are included; otherwise, the demanding party may reject the affidavit. 

Procedure for Correction of Name Affidavit 

- Type the information as listed above in an A4 paper and attach your password if required. 

- Take it to the registry section of a court or a notary public for attestation and stamping by the commissioner for oaths. Bear in mind that you will be charged a processing fee for it.

- You could be asked to confirm the accuracy of the information given in the affidavit, after which it will be signed, stamped, and issued to you. 

You may then present the affidavit of correction of name where it is needed. 

Note that sometimes, in addition to the affidavit of correction of name, you may be required to do a newspaper publication informing the public of the correction of name. This is done to avoid fraudulent name changes.
